Admissions
Click here to access the Pre-Admission Form for Calhoun Memorial Hospital. Complete this form and bring it with you to the Hospital to shorten the registration process.

Important papers you MUST have:
Medical insurance cards
Photo ID
Name and address of employer
Name, address and telephone number of closest family member
List of all medications you are currently taking
Advance Directive (Living Will) and Durable Power of Attorney

Visiting Hours
At Calhoun Memorial Hospital, we believe that family and friends play an integral part in the healing process and we welcome visitors. However, our patients' comfort and rest are a top priority so we ask that you respect our established visitng hours.
Visiting Hours are 7:00am - 9:00pm daily
Cafeteria
Friends and family members are invited to enjoy the delicious meals prepared by our staff.
There is a $5.00 charge for each meal and orders should be placed through the dietary department at ext. 2169.
Breakfast orders should be placed as early as possible, preferably by 7:00am.
Lunch orders should be placed no later than 9:30am and supper orders should be placed no later than 3:30pm.
The cafeteria is open from 7:00am to 6:00pm.
The dining room is always open and complimentary coffee is available.
Vending Machines
Vending Machines are located in the hallway by the laboratory.
Soda and snacks are available.
Telephone Service
Complimentary local telephone service is provided for your convenience. You may dial any extension in the hospital from a patient room and you may access an outside line by dialing 9. Free Wireless Internet is available for patients and guests at Calhoun Memorial Hospital and Calhoun Nursing Home.
Smoking Area
The designated smoking area is located in the parking lot in between Calhoun Memorial Hospital and R.E. Jennings Medical Clinic. Thank you for utilizing our designated smoking area.
Please alert the staff to any needs you may have during your stay with us. We will work to accommodate any reasonable request.
